Social Worker MSW – BH Care Management

Description Under general supervision of the manager of care management, the Social Worker is to work with and support the providers through the provision of resources, advocacy and referrals that address patient’s social determinants of health. The Social Worker is also an internal resource for community referrals and linkages.
