Providence is calling a Clinical Pharmacist Oncology Specialist (Pharmacy, Variable Shift, 1.0 FTE, 40 hour per week) to Providence Holy Family Hospital in Spokane, Washington.

In this position you will:
In addition to being able to perform the job duties within the Clinical Pharmacist job description, additionally the Clinical Specialist has the following job functions: Optimizing Drug Therapy: Identifies the need for, develops and implements medication orders, guidelines, procedures, care pathways, etc.

in conjunction with other disciplines to improve quality of care and outcomes of specific patient populations.

Initiates or directs initiatives in Pharmaceutical Care Services and interdisciplinary medication use quality improvement processes.

Makes recommendations to optimize medication use.

Identifies and implements cost containment approaches in the treatment of specific patient populations that control costs while upholding high patient-care standards.

Develops and participates in efforts to prevent medication misadventure, including adverse drug event and medication error reporting and analysis programs.

Participates in root cause analyses and resultant problem resolution as opportunities arise.

Pharmacy & Therapeutics: Assists the Advanced Pharmacy Practice Manager with Pharmacy and Therapeutic Committee activities related to practice area: Recommends, prepares and presents items related to specialty for the P&T agenda and attend meetings as needed.

Establishes and maintains a formulary based on scientific evidence of efficacy, safety, cost and patient factors.

Establishes plans and presents suitable educational programs for the organization’s professional staff on matters related to drug use.

Disseminates information on P&T actions and approved recommendations to patient care unit staff and physicians specific to specialty.

Required qualifications for this position include:
PharmD from an accredited college of pharmacy or Bachelor’s Degree in Pharmacy and 2 years clinical experience Post-graduate year 1 residency or equivalent educ/experience Required WA State Pharmacist and Preceptor License Must be active and in good standing Required Board certification in pharmacotherapy or appropriate pharmacy specialty area (related to practice area), must be obtained within 2 years of hire in area of specialization
Preferred qualifications for this position include:
Post-graduate year 2 residency (PGY2) or equivalent educ/experience Membership in national specialty organization Related to practice area 2 years hospital pharmacy and prior experience in respective specialty
