Founded in 1971, we believe that the quality of your health care should not depend on how much money you make, what language you speak or what your health is, because everyone deserves great care.Position Summary:Responsible for providing clinic-based nursing services providing direct age-specific and culturally appropriate care through the use of the nursing process, based on evidenced base care, serves as a clinical resource as a per-diem clinician.Your Contribution to the Team Include:Follows the registered nurse standards of practice to properly assess each patient to ensure appropriate medical care is delivered in the most optimal, cost effective, and culturally sensitive mannerIdentify and assess high-risk patients using established criteria, coordinate multi-disciplinary teams and resources to support patient care needs, provide care coordination/management for chronic medically complex or fragile patients across the life spanActively participates in the team care of patients with complex chronic diseases, coordinates care of complex patients, provides preventative health.Working closely with primary care teams, specialty care teams and medical providers, the per diem Nurse will establish a collaborative plan of care to assure adherence to the medical plan, improvement in functional status, and improved ability to self-manage.Collaborates with other health care team members regarding plan of care, utilization of clinical protocols variances in plan implementation, and expected goal/outcomes to optimize health which can include but not limited to:medication reconciliation,insulin management,Hypertension assessment,INR management, andinitial obstetrical pre-natal intakeAssess, provide recommendations and next steps for walk-in and telephone call triage patients evaluating acuteness of need and/or priority of intervention and document assessment information of all activities.Provide assessment, treatment and follow up related to basic wound care for patients with a range of complex medical conditions.Inform and follow up with patients related to abnormal lab test and x-ray results as needed.
