Our ideal candidates will enjoy being out in the community interacting with patients, caregivers, and families, and are excited to be part of a mission-minded and innovative team that wants to revolutionize the face of healthcare.In this position, you may:Provide geriatric and adult primary care to patients in your geographic area.

You’ll visit patients in their homes both in senior communities and their private residences where our patients reside to conduct the visits.Have autonomy in clinical decision-making with the back-up support of seasoned practitioners and administrative team.Duties and Responsibilities may include:Managing a primary care caseload of geriatric/internal medicine patientsManaging comorbid conditions such as diabetes, congestive heart failure and behavioral disordersIssuing appropriate orders, prescription refills and completing other administrative tasks post-visitUtilizing a comprehensive EHR to manage patient care.Collaborating with a remote medical assistant to manage caseload and address emergent issues and questions (M-F, 8am-5pm)Communicating effectively and promptly with team members, patients, caregivers and families via phone, email and faxMaintaining accurate and complete documentation of all clinical information and updating patient records in a timely mannerTraveling to Crown Health offices as required to complete administrative tasks and attend staff meetings and eventsRequired Qualifications:Physicians and Mid Level practitioners require completed degrees and certifications along with Master’s/Doctoral degree from an accredited Nurse Practitioner or accredited Physician Assistant program when applicable.Current NPI/DEA/PTAN numberActive license to practice medicine in WA stateReliable transportationCurrent and valid driver license and insurancePreferred Qualifications:At least one year of clinical experiencePrior experience with the geriatric populationCredentialing with Medicare and other major insurersWe Provide:Competitive PayFlexible and adaptable work provisions,Retirement and health benefitsCompany issued laptop with remote LTE access for enhanced team collaboration and efficient documentationBased on industry leading best practices –
