This is a Full-time position in Wauna, WA posted July 4, 2021.

Description: Providence is calling a Registered Nurse (RN) (1.0 FTE, Variable shift)) to support Transitional Care at Providence Mount St.

Vincent in West Seattle, WA.

The Licensed Nurse is responsible for planning, organizing, directing, and evaluating nursing services as part of the Interdisciplinary Team.

In collaboration with the Interdisciplinary Team, administers an individualized, comprehensive and Resident centered Plan of Care in accordance with current applicable Federal, State, and local standards, guidelines and regulations as may be directed by the Neighborhood Coordinator, Administrator, Director of Clinical Services, Primary Care Nurse or Resident.

In this position you will: Maintains shift accountability for resident care outcomes.

Administers total, comprehensive resident care to an assigned caseload of residents based on the nursing process and the interdisciplinary care plan.

Independently and interdependently plans, directs, supervises, and evaluates the nursing care of his/her caseload of residents.

Assists the Primary Nurse and team members in carrying out the resident plan of care: Considers the emotional, psychosocial, spiritual and physical aspects of resident strengths and needs.

Assures the residents needs and interests predominate in the planning, implementation and evaluation of nursing care.

Includes each resident and his/her family in all aspects of planning and teaching, giving consideration to the individual residents strengths and needs.

Maintains confidentiality of all resident information, assures that residents are treated fairly with kindness, dignity, and respect.

Also assures that resident choice, independence, and privacy are maintained.

Qualifications: Required qualifications for this position include: Degree in nursing.

1 year Education & Training: LTC geriatric experience.

5 Center Primary Nurse: Teaching experience.

Licensed nurse licensed by the State of Washington.

Education & Training: Current CPR certification (New Hires must obtain within 30 days of hire).

Food Handler’s Permit (or must obtain within 14 days of hire) Preferred qualifications for this position include: In Long Term care.

Supervisory experience with nursing assistants (NACs).

Experience with an interdisciplinary team.

3 years 5 Center Primary Nurse: direct care experience.

5 Center Primary Nurse: MDS/RAI experience.
