The Vancouver Clinic: Social Worker – Licsw Long-Term Care

This is a Full-time position in Salmon Creek, WA posted July 4, 2021.

We are looking for a LICSW to provide therapy services with a focus on our Long-term Care patients’ needs.

The primary role of the Social Worker Mental Health is to work in close partnership with the Vancouver Clinic psychiatry team and primary care providers, supporting and coordinating the physical and mental health of patients on an assigned caseload.

The Social Worker Mental Health serves as an integral member of the healthcare team providing assessments, coordination, care planning, information and potential referrals to community resources.

The Social Worker Mental Health delivers age-appropriate therapy and care to a diverse socioeconomic population of patients and their caregivers in accordance with policy and procedure and state and federal regulations.

EDUCATION AND SKILLSREQUIRED:Must have current unrestricted license as a Licensed Independent Clinical Social Worker (LICSW) in Washington State.Must have minimum of two (2) years’ experience in the use of psychosocial assessment and psychotherapeutic modalities for common mental health disorders and substance abuse disordersMust be able to effectively engage patients in a therapeutic relationship.Must have excellent written and verbal skills, negotiation, delegation, time management, problem solving and customer service skills both in person and by phone.Must have strong counseling, assessment, diagnostic, intervention, and treatment planning skills with a diverse population.Must have working knowledge of evidence-based psychosocial treatment and brief behavioral interventions (ie motivational interviewing, problem-solving treatment, behavioral activation, etc.)Must have basic knowledge of psychopharmacology for common mental health disorders.Must be computer-literate and, preferably experienced in automated clinical information systemsMust be able to thrive in a remote work environmentMust demonstrate customer-focused service skills; patient centered, positive problem-solving advocate.Must demonstrate ability to effectively and efficiently handle demanding workload involving multiple tasks.Must demonstrate ability to function independently as a collaborative, supportive team member.Must demonstrate flexibility within a dynamic and ever-changing environment.Must demonstrate ability to work collaboratively within a multi-disciplinary team to provide support, connection and coordination.

Preferred:Three to five (3-5) years of social work experience, previous experience with mental health social work focus preferred.Knowledge of chronic and acute disease and how it impacts functioning preferred.Expertise with short and long term counseling interventions preferredEpic experience highly preferred.
