This individual works with providers, peers, community colleagues, and others to orchestrate care across the health care continuum, to identify opportunities to continually improve patient care and services, to improve population health, and to achieve collaborative practices that exemplify MultiCare Health System’s commitment to patient centered care and community engagement.Minimum Requirements:3 years Hospital or equivalent experience; partial experience prior to obtaining a Master’s degree may be consideredHealth industry knowledge preferredHealthcare financial assistance knowledge preferredLeadership experience a plus Education/Licenses/Certifications:Master’s Degree in Social WorkLicensed in the state of WashingtonLicensed Independent Clinical Social Worker (LICSW); OR Social Worker Associate Independent Clinical (LSWAIC) with a commitment to obtain their LICSW within 4 yearsMultiCare is an equal opportunity employer.
