Palliative Care Social Worker MSW – Hospice Snohomish

Providence is calling a Palliative Care Social Worker MSW (Hospice Snohomish, Days, 1.0 FTE) to SCS Hospice in Everett, WA.

– especially those who are poor and vulnerable.In this position you will:Assesses psychosocial needs for specific patient populations using advanced practice palliative care and hospice knowledge and skills:Obtains psychosocial history Identifies patient short and long term goalsIdentifies available family and social support Initiates advance care planning discussionIdentifies discharge options and ongoing care needs related to patient diagnosis, prognosis and identified care giving needsProvides critical psycho-social support to palliative care patients and their families through the palliative care services consult teamUses their professional background and skills to assist the team and facilitate palliative care patient-family meetings, and assists team, attending physicians, staff, and patient and family in the clarification of the palliative goals and plan of careAssists the Palliative Care Team in ensuring excellent palliative care patient care services, coordinating educational and outreach services, leading quality improvement initiatives, and linking team efforts with other health system departments including continuum and community servicesSynthesizes information from the psychosocial history and needs assessment to develop, document and implement the psychosocial dimension of the holistic palliative care treatment plan Develops and implements treatment plan in collaboration with members of the Palliative Care Team, Collaborates, as needed, with patient’s primary physician, consulting physicians and other health care team members in developing realistic treatment goals.Communicates effectively with patients and families for understanding of diagnosis, prognosis, and plan of careRequired qualifications for this position include: Master’s degree in Social Work 3 years experience in Inpatient, Outpatient, or home-based Social work Demonstrated evidence of experience in palliative care settings, such as hospice, i.e., ability to: Provide and model interdisciplinary palliative assessment and management of physical, psychosocial and spiritual symptoms during aggressive treatment and end of life care.

Work independently as well as collaboratively on a small interdisciplinary team Current licensure as a Social Worker in the state of Washington Current Healthcare Provider CPR certification Valid Washington State driver’s license and access to daily use of an insured, reliable vehicle or an alternate ability to travel to sites Proof of current auto insurance.

Preferred qualification for this position include: Working knowledge of research techniques, data collection, and policy, program and curriculum developmentExperience with quality improvement methodology LCSW or equivalent About the ministry you will serve.Providence Hospice of Seattle is a not-for-profit, community-based hospice ministry serving patients in their places of residence throughout King County in Washington state.
