Era Living is seeking a Community Health Nurse (LPN/RN) to become the newest member of our Community Health team in Seattle, WA. As part of our On-Call Nursing Pool, you must be available to work a minimum of 8 hours per month and may be called to work at any of our eight communities (Aljoya Mercer Island, Aljoya Thornton Place, Ida Culver House Broadview, The Gardens at Town Square, University House Issaquah, The Lakeshore, Ida Culver House Ravenna, and University House Wallingford.)

Primary Responsibilities:
The Community Health Nurse will contribute to the assessment of residents, implement health care services and collaborate with the Community Health Director to provide health promotion and monitoring of residents.
Knowledge, Skills, and Abilities:
- WA State RN or LPN.
- Minimum 2 years’ experience working with older adults; nursing experience in an assisted living facility preferred.
- Able and willing to travel locally.
- Excellent English communication and interpersonal skills.
- Computer proficiency.
- Ability to pass DSHS background check, required.
