– 4:30p You will have the excitingopportunity to develop a new Social Worker role in our Heart Institute working across the continuum of care from Regional Medical Centers to Downtown Clinics to acute care.Workingcollaboratively with RNs,support staff and providers you willaddress patients” social needs, provide case management as well as providing short term therapy.

The Social Worker provides social work interventions forMedical Center patient and family needs.

Some responsibilities include but are not limited to: coordinating discharge plans and/or care plans for in/outpatients, including developing plans for ongoing treatment through services and agencies; performing a psycho/social assessment for continuity of care planning for in/outpatients; providing short-term adjustment counseling to assist with emotional, social, and economic stresses of illness and disability.

Qualifications: Master”s of Social Work from an accredited college or university is required.

Registration with the state of Washington as either a Licensed Social Work Associate (LSWAIC), Independent Clinical Social Worker (LICSW), Social Work Associate Advanced (LSWAA) or Social Work Advanced (LISW) May consider an Agency Affiliated Counselor with the expectation of active pursuit of their Social Work Licensure.

Strong interpersonal and organizational skills Working knowledge of individual and family dynamics and the ability to perform psycho/social assessments Understanding of systems and how to work within systems Understanding of the impact and implications of disease and the course of disease Virginia Mason offers much more than competitive compensation and benefits.
