Providence is calling a Social Worker (MSW) (Hospice Seattle, Days, 1.0 FTE) to SCS Hospice in Tukwila, WA.
*This position is offering a $2,500 sign-on bonus to eligible candidates.*

In this position you will:
Assess initially and on ongoing basis the needs of terminally ill hospice patients and families, addressing psychosocial issues.
Collect and complete initial psychosocial assessment, including information from range of available sources – patient, family, IDG, community agencies, etc.
Interpret assessment data to identify problems and potential problems/needs.
Orient patient/family to Hospice services, pt/family rights & responsibilities, overall financial coverage issues.
Develop plan of care with the Hospice IDG which is: Respectful of and consistent with patients’/families’ goals and values; Inclusive of the perspective of other team members; and geared towards maximizing patients quality of life and comfort, promoting patients’ and families’ preparedness, and preventing crises.
Appropriately collaborate with Hospice IDG, including attending physician, to create and update the plan of care and to optimize the care provided to patient and family.
Qualifications:
Required qualifications for this position include:
Master’s degree in Social Work (MSW) from an accredited School of Social Work
1 year Post-Masters work experience in a health care setting
1 year prior internship experience in a Providence Health System hospice or palliative care setting may be substituted for the required one year post-Masters work in a health care setting
Current Social Work Certification or License through Washington State Department of Health
Current Healthcare Provider Cardiopulmonary Resuscitation (CPR) certification
Current Washington State Driver’s license and automobile insurance
