Care Coordinator

EverHome HealthCare is a well-established and highly respected Home Health and Home Care agency serving the Central Puget Sound. We are looking for an experienced individual to join our Clinical team. Care Coordinator is responsible for day-to-day tasks to assist the clinical department. Ideal candidate is a Certified Nursing Assistant with office expertise, Medical Assistant who can obtain their CNA, LPN, clinician, nursing student, or planning to go to nursing school. A critical component of the role of the Care Coordinator is to ensure that all who come in contact with our company see EverHome HealthCare as having a culture of compassion, accountability, results, and expertise, with a strong emphasis on customer service. Top 6 Responsibilities and Accountabilities include: Ensure client satisfaction and retention through constant communication and frequent home visits with clients, caregivers, and responsible partiesAssist in caregiver success through training and coaching, reviewing plan of care with caregiver, and doing supervisory visitsDo client opens, write clients plans of cares, and enter in Clear CareAttend first shift with new clients to introduce caregiver to client and ensure smooth start-of-care. Start-of-care times may include evenings and weekends.Out of a 40-hour work week happily work up to 20 hours of caregiving shifts with availability to be on-call 2 nights a week and 2 weekends a month for caregiving shiftsAssist Clinical Director to ensure organization and attention to detail in department workloadQualifications: Bachelor’s DegreeMedical Assistant, Clinician, LPN, or CNA. If MA, clinician, or going to nursing school able to obtain NAR/HHA-C upon hire and CNA within 90 days of employment.Previous experience in health care related fieldExcellent customers service skills and communication skills both verbally and writtenProficient on Microsoft office programs and able to learn new software programs as neededGood judgment with the ability to make timely and sound decisionsCommitment to excellence and high standards along with strong teamwork skillsReliable transportation and clean driving recordExtremely organized and detailed orientedVersatility, flexibility, and a willingness to work within constantly changing priorities with enthusiasm and a client comes first attitudeBenefits: medical, dental, vision, PTO, and 401K Job Type: (1) Full-time or (2) part time employees Salary: DOE Job Types: Full-time, Part-time
