Providence St. Joseph’s Health: Rn – Hospice Sound

Description: Providence is calling a Registered Nurse (RN) (Hospice Sound, Days, 1.0 FTE) to SCS Hospice in Lacey, Washington.

This position involves certified nursing work managing the plan of care for Hospice patients.

This position provides direct health care services to Hospice patients and their families in their homes, and must demonstrate independent clinical expertise throughout the nursing process.

This position works collaboratively with members of the interdisciplinary hospice team, and oversees the work of appropriate caregivers (such as Hospice Aides and/or LPN) to ensure quality patient care and to enhance the quality of life of dying patients.

This position is supervised by a Hospice Clinical Manager, through patient care documentation and electronic medical records, written and verbal communication, and direct observation of patient care.

Shift, schedule and caseload may vary depending on position assignment.

In this position you will: Nursing care needs are assessed as needed for a defined group of clients; after a referral, an accurate history and physical exam will be performed; documents assessment data within 24 hours of home visit.

Determines priorities of client care based on client’s needs and available resources; manages time effectively; prioritizes client’s needs effectively.

Provides information to client & significant others regarding diagnosis, interventions & available resources.

Maintains open communication to client & significant others.

Assesses, plans, implements & evaluates educational needs and readiness of client & significant others to learn; develops individualizedteaching plan tailored to the client’s goals; implements & documents teaching plan; provides concise teaching materials & practical technique demonstrations that are for the home setting; modifies plan after evaluation process as needed; provides or assists patient and significant others in obtaining equipment & supplies necessary to provide quality care; encourages compliance with therapeutic nursing and medical regimes.

Qualifications: Required qualifications for this position include: Graduate of a school of nursing 2 years nursing experience required Current Washington State Registered Nurse License Valid Washington State drivers license, insured and reliable transportation Healthcare Provider BLS/CPR certification Preferred qualification for this position include: 1 year hospice, home health, skilled nursing facility, medical/surgical, or oncology.
