RN Team Based Care – Per Diem

This is a Contract position in Okanogan, WA posted July 21, 2021.
RN Team-Based Care works with the medical team to provide quality, comprehensive care to patients with complex health situations. Assist team with the identification of complex chronic diseases, coordinate care with team and patients, interact & follow up with hi-risk patients regarding care.
Required: WA State RN license/certification
I understand that FHC is obligated to provide a drug-free workplace in order to receive state and federal grant funds and that I will be required to pass a drug test before a final offer of employment is made.
