JOB SUMMARY
The Team Leader RN/LPN is responsible for coordinating the total care of a designated number of residents on their assigned unit and shift.
The RN/LPN is also responsible for evaluating and supervising the Certified Nursing Assistants who perform direct nursing care of the residents.
This position promotes harmonious relationships among residents, resident families and nursing personnel, and other health practitioners.
The Team Leader RN/LPN keeps the RCM/RCC appraised of all problems occurring on the assigned unit keeping with Wesley Homes Health Center’s policies and procedures.
ESSENTIAL FUNCTIONS
At the start of each shift listen to taped/verbal report and give the report to the NAC’s prior to the start of patient care duties.
Makes rounds to observe and evaluate the physical and emotional status of each resident at the beginning of each shift and throughout the shift as needed.
Alerts the RCM/RCC to resident needs that require immediate attention. Assigns the responsibility for the direct care of a specific group of residents to the Certified Nursing Assistant (NAC) based on the needs of the resident and the needs of the facility.
Assures that the NAC completes resident care activities in a timely, efficient, and appropriate manner.
Assists in problem solving of resident care needs with the resident and NAC.
Makes recommendations to the RCM/RCC and Staff Development when there are areas of concern regarding NAC’s understanding of resident care practices. Supervises NAC’s that are performing direct resident care to assure that the resident’s needs are preferences are met.
Performs performance feedback, evaluations, and corrective action with appropriate staff as assigned when needed. Administers medications and treatments in a timely manner and documents as appropriate.
Monitors response to medications and treatments and communicates to the physician when necessary.
Orders and re-orders medications and supplies when necessary. Other duties as assigned (Performs narcotic count each shift with the on-coming shift; responds to, evaluated, treats, investigates and documents resident accidents/incidents; documents all resident assessments and treatments; supervises the administration and documentation of dietitian ordered nourishments, supplements and general hydration; etc)
QUALIFICATIONS/REQUIREMENTS
Education Background
High School Diploma or equivalent 2-year college degree in Nursing or related field
Licensure, Registration, Certification
Current LPN or RN license in Washington State. CPR Certification.
Special Training/Skills
Knowledge of State and Federal regulations as they relate to long-term care facilities. Demonstrated knowledge and ability to perform clinical responsibilities. Outstanding communication skills with the ability to speak, interpret, and understand conversational English. Computer skills to include experience in MS Office (Word, Outlook) and Electronic Health Records systems. Maintain professional competency by independently attending continuing educational offerings.
Work Experience
Previous Nursing experience in a Skilled Nursing Facility. Experience with IV’s, maintaining pain infusion pumps, and feeding tubes. Experienced in supervising a staff of multiple employees.
Physical Demands
Ability to stand/walk 4 – 6 hours per day. Ability to sit for long periods of time for charting, phone consulting, data entry. Ability to lift up to 50lbs on occasion. Ability to bend, stoop, and squat frequently for assessment of residents.
INFECTIOUS RISK CATEGORY
HIGH RISK: Likely to have frequent occupational exposure with patient’s blood or other body fluids.
May require protective equipment to be worn during some aspects of direct patient care.
SOME (COVID-19) RISK: Possible occupational exposure to residents or clients with COVID-19.
May require personal protective equipment (PPE) to be worn during some aspects or seasons of job.
