Position Summary:
Full-time Case Manager available for our Care Management Program located in Yelm. The Case Manager or Interdisciplinary team member is responsible for providing program support, community outreach, and case management for community health center patients receiving care management services.
The Care Management Case Manager has a basic understanding of how to help clients who have both physical and mental health needs, and understands how to promote a healthy lifestyle and promote patients ability to choose positive and healthy outcomes. May assist with case management, outreach and engagement, and technical support. We are looking for someone who has the capacity to understand the impact of chronic disease and working with social determinants that impede self-care. There will be additional training after hired.
Education and/or Requirements:
- High School Graduate, GED or equivalent
- Must have two years social service work or have done similar work.
- Experience with MS Outlook and electronic data management.
- Bilingual in English/Spanish preferred but not required.
