Full Job Description

To provide management and leadership for the resident care team members through planning, organizing, developing and implementing all phases of nursing, medical records and social services policies and procedures. To set the standards of resident care through supervision and example.

* Coordinates wellness care program with resident, family, physician, consultants and other community departments for total resident care.

* Visits residents daily to insure maximum care and to ascertain need for additional or modified services.

* Supervises maintenance of records of wellness and medical treatments and related services executed by resident care team.

* Insures availability of supplies and equipment related to wellness care.

* Monitors resident incident reports and medication error reports to assess causes and preventative measures in injury control.

* Acts as liaison between community Wellness department, RN Consultant, Internal Consultants, and physicians.

* Organizes and ensures completion of Community Quality Monitoring, including but not limited to fall tracking, Hospital readmissions, med errors, etc.

* Develops plans of action for the nurses and caregivers based upon resident need using existing wellness software.

* Establishes staffing levels for Wellness Department and recommends levels to Executive Director and then schedules resident care staff on all shifts, according to budget guidelines.

* Suggests changes to policies and procedures for the Wellness Department, including wellness, restorative and medical records and recommends changes to Wellness Consultant.

* Monitors Wellness budget, including labor.

* Coordinates between wellness and all other departments.

* Completes resident assessments on time. Culture:

* Acts as profession leader, role model and innovator in the community. Leadership:

* This position manages team members in the Wellness and Memory Care department. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

* Keeps Executive Director informed of resident care activity, team member concerns, staffing patterns and other relevant topics. Records and submits to Executive Director and acts on all routine problems.

* Welcomes and interacts with newly arriving residents to achieve a successful move-in and content resident/family.

* Possesses knowledge of Executive Director’s duties and responsibilities, which assure continuity of administrative duties in absence of the Executive Director. Wellness:

* Acts as directed by Executive Director. * Studies and analyzes techniques in resident care, which will motivate and move residents towards greater independence, as well as higher levels of functioning.

* Trains all community team members in techniques developed to maintain residents’ independence and higher functioning. Documents training and conducts refresher training session to maintain appropriate skill set. Evaluates team member skills by observation and implementing corrective action where necessary.

* Coordinates with and supports the Life Enrichment Director to achieve an exercise program.

* Evaluates individual resident care requirements prior to and upon move-in to determine care needs.

Supervisory Responsibilities: This position manages team members in the Wellness and Memory Care department. Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Must meet all state health requirements including T.B. and must be able to pass states perspective background clearance

Education/Experience: Five years’ experience in nursing techniques, including two years in long term care. Assisted living experience preferred. Demonstrates leadership skills in applying nursing and rehabilitative care techniques. Must have in-depth knowledge of State and Federal Rules, Regulations, and Standards.

Language Skills: Read and comprehend simple instructions, short correspondence and emails; write simple correspondence; effectively present information in one-on-one situations and small group situations to customers, and other employees.

Math Skills: Add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals; compute rate, ratio and percent and to draw and interpret bar graphs.

Reasoning Ability: Apply common sense understanding to carry out detailed but uninvolved written or oral instructions; to deal with problems involving a few concrete variables in standardized situations. Ability to identify problems and escalate them.

Computer Skills: Proficiency with computer usage, and programs including, Microsoft word, Excel, Outlook/E-mail. State Requirements: Any state required training/certification for this position.

Physical Demands and Work Environment: The physical demands and work environment described below represent the activities and surroundings of the positions. Frequently required to lift up to 10 lbs. when lifting resident medical files and supplies. Constantly required to lift 11 to 25 lbs. when stocking medical supplies up to 15 lbs. Occasionally may be required to carry 11 to 25 lbs. during course work when stocking medical supplies or medication/treatment cart. Occasionally may require reaching above shoulder height when reaching for medical charts on shelves. Reach at shoulder height: Frequently required when reaching for medical supplies on shelves. Occasionally required to reach below shoulder height occur when reaching for supplies or information in drawers. Occasionally may be required to push/pull if medicine cart needs moving. Typically rolls easily over carpet or tile, weighing approximately 200 lbs. Constantly grasping pen/pencil, answering phones, may need to use a computer. Work Environment: The noise level in the work environment is usually moderate, consisting of usual business office sounds including but not limited to computers, printers, telephones, light foot traffic and background conversation.
