RN PACE Care Manager

Providence is calling an RN PACE Care Manager – Gresham Nursing Services (1.0 FTE, Days) to Providence Elderplace Oregon in Portland, OR.

We are seeking an RN PACE Care Manager – Gresham Nursing Services who will provide care management, care coordination, skilled care and services for at-risk frail elder patients with complex medical and psychosocial issues. Population-based disease management approaches and application of clinical protocols are integral to the program. This position is a core member of the patient centered PACE interdisciplinary team (IDT). The goals of care management are to assure a systematic approach to the coordination of health services for patients along the care continuum in order to improve the patient experience, contain health expenditures and improve quality. Clinical Care Managers assist providers in effectively identifying needs and coordinating access to appropriate care and services in an effort to improve or maintain the patient’s functional status and independence. This position is responsible for the planning, overseeing and monitoring of all nursing and unlicensed caregiver services to each patient on their panel within the context of an IDT. This position requires travel between multiple locations.

In this position you will have the following responsibilities:

  • Perform comprehensive nursing assessments to develop an evidence-based nursing plan of care that, as part of the interdisciplinary participant directed plan of care, contains specific, attainable and measurable goals appropriate to a community setting and across the continuum of care
  • Participate as a member of the interdisciplinary team for patient care planning and decision making
  • Serve as a clinical liaison to participant caregivers, including family members and staff of the PACE contracted network
  • Provide caregiver education tailored to the specific needs of the individual
  • Provide mentoring, education, anticipatory guidance and nursing delegation to community caregivers and other team members in the application and implementation of the nursing care plan appropriate to the caregiver’s scope of practice
  • Act as a patient advocate to ensure that needed health services are provided in a timely manner. Identify, research and/or refer potential quality of care issues
  • Monitor and reassess the plan of care to ensure that quality, quantity and goal achievement are appropriate and cost-effective
  • Consistently focus on improving nursing care quality by readjusting care strategies and priorities
  • Maintain up to date clinical expertise and care management skills
  • Contribute to ongoing process improvement related to procedures, policies, patient support and documentation tools
  • Advocate and provide care coordination and management services for patients with identified needs
  • Act as a liaison with other members of the IDT and contracted services
  • Perform skilled nursing procedures and treatments according to accepted clinical program standards
  • Document ongoing follow-up and monitoring of effectiveness of the interventions and reinforce the treatment plan

Qualifications:

Required qualifications for this position includes:

  • Current state of Oregon RN licensure
  • Current Basic Life Support (BLS) at time of hire
  • American Heart Association Basic Life Support (AHA-BLS) for Health Care Provider certification within six (6) months of hire
  • Two (2) years of RN experience in PACE, home health, skilled nursing facility, community health, hospice nursing or related experience

Preferred qualifications for this position includes:

  • Bachelor’s Degree in Nursing
  • Current recognition by a national professional organization in care management, geriatrics or hospice
